The TST Support Services Approach
TST support registration manager carries out assessments on proposed service users. This involves users, family and their trained caregivers. When this is done, a transition plan is prepared that's looks at urgent needs, risks, past issues, good procedures that has worked for the user in the past and the handling of care to local carers.
Having in mind their needs, users bits by bits get introduced to the service and then transfer to local care will be completed. This includes Gp services and domestic health services where needed. Tst support staff will conclude a plan with the user giving the best support through the procedure.
We are devoted to measuring results, we let users know on their improvements and help them focus on that which is important. These measures are vital when developing their personal plan.
