It is assumed that a person is discharged from the hospital things get back to normal. Sadly, it isn’t always like that infact, the first month after discharge is said to be a period of high risk continuous care and support is further adviced for seniors going home including a non-medical home care.

Discharge is not made until the necessary home care facilities and equipment needed has been provided to support service users. All of our Respite/Holiday care program promises to carter for the needs of all our service users even in the absence of our regular care givers.

How am I assessed for homecare support?

  • Once an individual is in need of further home care and support, all required support and home adjustment is made ready by staff before discharge. During the cause of these arrangement and it is discovered that the user will need personal care or assistance, a contact about care plans and services will be given. If the user have a carer, then they are also entitled to a separate care assessment and there may be support available for them too.
  • Once a user is qualified for a home care service, there will a finance check on the individual to see if he is eligible for financial support. If yes, user’s local authority will be to prepare services and if found not eligible, user will have to arrange their own services to meet their needs.

Can I arrange my own home care after leaving hospital?

Home care services can be provided whether you are found eligible or not, perhaps a user may need help with domestic chore for some time during recovery period. Staff may suggestion local organizations to help out. These local voluntary organizations may support with non-medical tasks.

What support can I receive when I get home?

When a user leaves for home and support has been arranged, there are constant checks by social services for a period of time to see if the support is the right for the user. After which care plan should be reviewed at least once a year or more often if needed. In case of a user living in the care home permanently after being discharged from the hospital, intermediate care and reablement services should have been considered. Individual is allowed to fully recover before making such decision but if the staff thinks this will hasten individual’s recovery then it will be considered.

After a hospital stay, we understand you want to heal and get back to your home and best possible health as soon as possible. Recuperation from illness doesn’t end when a person walks out the hospital doors, it most likely takes weeks or months to fully recover while diligently following the doctor’s directives.

Nobody plans to visit the hospital anytime soon after being discharged. In order to avoid being readmitted, provision for all the help they might at home is made before arrival. In-home care after a hospital discharge ensures that your loved one receives the exact support they need to increase their chance of a full recovery with constant support, check-in and monitoring.
